ENTRY PROCEDURE: Contestants shall apply on official OGA entry blank or online at www.ohiogolf.org. Entry fee refund available only for medical reasons after deadline date, June 6, 2022, 11:59 p.m. All refunds are subject to a $15 service fee. Late or incomplete applications not accepted. No telephone entries. On-line entry is available at WWW.OHIOGOLF.ORG.
APPLICANTS: Must be (1) a male who has not reached their 19th birthday on or before June 15, 2022. (2) a WHS Handicap Index of 10.4 or less (3) a resident of the State of Ohio or in the case of a minor, his parent/legal guardian must be a resident. In order to pay the member fee, entrant must have an active WHS Handicap Index at an OGA member club. A list of member clubs may be found at www.ohiogolf.org. A resident is defined as meeting four (4) of the nine (9). 1. Ownership or leasing of Ohio residence 2. Proof of payment of utilities at property for which residency is being claimed 3.Ohio homestead exemption declared on residence 4. Place of employment in Ohio 5.Ohio Driver’s License 6.Ohio Vehicle registration 7. Ohio address listed on federal income tax return filed by player 8. Has a WHS handicap at a golf club in Ohio. 9. Voter registration in Ohio.

REGISTRATION: All players are required to register at Maumee Bay State Park GC, Monday, June 13, 2022. A player may call or email the OGA to pre-register, if he is unable to get to Maumee Bay State Park GC on Monday. If a player fails to register, his spot may be given to an alternate.
PRACTICE ROUND: Monday, June 13 Entrants will be sent a link via email to book their own tee time through the event portal. Please do not call Maumee Bay State Park GC to book a tee time for June 13.
CADDIES: A player may not have a caddie during the practice round or any tournament round.
PARENTS: Parents must remain a reasonable distance from play, at least 50 yards. Parents are not allowed on any teeing area or putting green during practice or tournament rounds. After the first warning, the player will be disqualified.
SHOES: During a round, a player must not wear shoes with
• Traditional spikes—that is, spikes having single or multiple points designed to penetrate deeply into the surface of the ground (regardless of whether made of metal, ceramic, plastic or other materials.
• Spikes of any design that are entirely or partially made of metal, if such metal may come in contact with the course penalty for breach of this Local Rules: See Rule 4.3. Modern Local Rule G-7.
TRANSPORTATION: (Ohio Amateur and Junior) During a round, a player or caddie must not ride on any form of motorized transportation except as authorized or later approved by the Committee. See Modern Local Rule G-6 for additional information and Penalty.
